You can either fill out a digital form or download a form in PDF format so you can print it out.

If you’re using the digital form option, please make sure you read through the questionnaire first and make sure you have put aside enough time to fill it out as there isn’t an option to save it. I’d recommend 30-60mins depending on how much you want to take your time with it.
For both options, you will need to take some measurements so please make sure you have a flexible tape measure handy.
It is optional to attach baseline photos of yourself (front/side/rear in either a bikini, bathers or tight fitted gym clothing so that Dayni can assess your body composition to help design your program).
If you’re choosing the digital option and wish to attach photos, this can be done after you’ve submitted the form. You will be taken to a page after you’ve submitted the form, click on “attach photos here” which will link you to your email account so you can attach your photos and they’re sent straight to Dayni through your email account.
